How do I become an in-network provider with MBHP?
Please review MBHP’s credentialing criteria for network consideration. Once you have determined that you or your organization meets the minimum credentialing requirements, please visit the Carelon Behavioral Health website and follow the prompts to complete a request to join the network.

To ensure your request will be reviewed and processed timely, please make sure the following documents are submitted:

If you are not on CAQH, you will need to create a ProView account and give Carelon Behavioral Health permission to view and process your application.

MBHP promotes the online universal credentialing process offered by CAQH at no cost to the provider.  This service helps providers reduce credentialing administration and paperwork. Please access the CAQH website, then review your application for completeness, upload required documents, and ensure your attestation is current.

What services does MBHP cover, and what codes can I bill with?
The MBHP Benefit Service Grid gives you the most current information on the services that MBHP covers and the correct service codes and modifiers that should be used when billing these services.

Where can I direct Members to learn more about MBHP services?
MBHP has a new Member Portal where Members can access information on covered services, helpful resources, and wellness articles.

Where can I get information on the payment rate for a specific service?
All contracted services and their payment rates can be found in your Fee Schedule. To request a copy of your Fee Schedule, please email provider.relations@carelon.com.

How do I change/update my address?
The request for all address changes or request to add an additional location has to come from the practitioner. Please email provider.relations@carelon.com for address changes, address terminations, pay to address, W-9, and additional locations.
