PCC Site Visit Materials

• Help PCCs to learn about and access behavioral health resources for Members on their panels
• Conducts regionally-focused quality forums in clinical and administrative areas
• Develops and distributes a variety of support materials, with a particular focus on supporting PCCs in the use of care management and behavioral health services

Support PCCs in Use of Care Management Services

Integrated Care Management Program (ICMP)

• Provide overview of ICMP including referral process
• Review reports with practices related to use of ICMP and opportunities to increase referrals
• Work with practices and ICMP to improve communication regarding Members enrolled in ICMP
• Workwith ICMP team and PCC practices to identify and engage with additional resources that may be useful to addressing social determinants of health
• Use high-risk report to identify individuals who are not enrolled in ICMP but may benefit from care management services
• Work with ICMs to coordinate in-person visits with PCC practices

Increase PCC Knowledge of Behavioral Health (BH) Services and Support Primary Care and BH Integration/Linkages

Improving Access

• Educate PCCs about MBHP's contracted BH services, including outpatient, Community Support Programs (CSPs), Emergency Services Programs (ESPs), substance use disorder (SUD) providers, and Children's Behavioral Health Initiative (CBHI) services available to Members
• Identify service providers in the area to identify and create referral pathways
• Promote Massachusetts Child Psychiatry Access Program (MCPAP) and MCPAP for Moms services
• Educate and promote the use of Massachusetts Consultation Service for the Treatment of Addiction and Pain (MCSTAP)
• Inquire, educate, and promote the screening, treatment, and referral of SUD and Medication-Assisted Treatment (MAT) prescribing for individuals with opioid use disorder; Provide information and assistance related to waiver process and/or coordinating with MBHP director of Addiction Services

Support Integration

• Educate PCCs regarding the process of establishing primary care behavioral health services within the practice (e.g., credentialing and regulations)
• Assist PCCs in identifying BH providers interested in co-location/integration when possible
• Discuss and educate PCCs on relevant and approved screening tools for MassHealth Membership
• Utilize Top 5 Behavioral Health Providers report as a tool to support integration with commonly used BH providers of PCC's PCC Plan panel
• Promote and educate PCCs on the use of the daily inpatient psychiatric admission report.

Resources and Training

• Provide PCCs information on upcoming PCC Plan/MBHP trainings, webinars, and forums
• Educate and provide resources to PCCs including website links and materials
• MassHealth Training Forum

• MassHealth issues provider bulletins as needed to communicate procedures, reminders, and other information to MassHealth providers.
• The Physician Manual guides physician providers to the regulations, administrative and billing instructions, and service codes they need.
