Health and Wellness Library
MBHP has an online Health and Wellness Library on a website called Credible Minds.
This health library is a resource for both providers and Members. For
providers, the Health and Wellness library offers resources on a wide variety of health conditions that can be printed
and given to Members and content that can
help patients handle difficult-to-address concerns such as bullying.
Providers can direct Members to the Health and Wellness Library to
promote self-management of health conditions and adherence to treatment
plans. Clinical content in the library is evidence-based, written by
subject matter experts, and reviewed on an annual basis. Just a few of
the library topics include:

- Smoking and tobacco use cessation
- Anxiety
- Stress
- Managing stress
- Therapy and Support Groups
- Depression
- Postpartum Depression

Through the Health and Wellness
Library, Members can access comprehensive, integrated information
related to physical and behavioral health, wellness, and more. The site
includes interactive assessments, videos, podcasts,
and other tools. The Health and Wellness Library encourages Members to
become active participants in their healthcare and to make healthy

Provider Connection
Formerly named the Primary Care Clinician (PCC) Plan Quarterly, Provider Connection is an integrated
newsletter featuring news, resources, and information benefitting both PCCs and behavioral health providers. Provider Connection
contains stakeholder viewpoint columns, policy updates from MassHealth,
and tips to increase practice efficiency and tackle common challenges.
It also functions as a forum for providers of all types and sizes to
share best practices and research ideas. Additionally, Provider Connection offers listings of events and trainings relevant to both PCCs and
behavioral health providers.

Carelon/MBHP Provider Toolkit
The Carelon/MBHP Provider toolkit is reviewed and updated annually to reflect most recent resources. The Carelon/MBHP Quality team in collaboration with Medical, Clinical, Peers, Provider
Relations, and other stakeholders across the organization revise the online resource. The Provider toolkit is intended to support primary care clinicians and behavioral health clinicians
by providing a quick guide to behavioral health references. The toolkit is also a great resource for behavioral health providers and our health plan partners. The toolkit is useful for
managing populations with co-occurring disorders and promotes an integrated healthcare approach encouraging whole-person health by providing PCPs and other practitioners resources they can
use with the Members they serve. The toolkit includes resources for the management of depression, substance use disorders, anxiety, and schizophrenia.

PCC Tool Kit: Substance Use Disorder Resources
PCCs and other health care practitioners working in primary care settings
play an important role in providing comprehensive, integrated care to
PCC Plan Members. This care can often include helping Members manage
mental health and substance use disorders. To assist PCC Plan providers
working with Members on these issues, the PCC Plan and MBHP have
developed a tool kit that includes educational material and information
about available services for Members seeking substance use disorder
treatment. The tool kit is available for download below. Once you

Primary Care Behavioral Health Screening Tool Kit
As part of the MassHealth Children's Behavioral Health Initiative (CBHI), the PCC Plan, in conjunction with the Massachusetts Behavioral Health Partnership
(MBHP) and the Massachusetts Child Psychiatry Access Program
(MCPAP), has published a behavioral health screening tool kit for
primary care providers who provide care for children under the age of
21. CHBI requires PCCs to use an approved, standardized behavioral
health screening tool selected from the list which can be found in the PCC Plan Health Education Materials Catalog (item number CBHI-Table)
or on the CBHI website.
This tool kit includes guidelines on general implementation of behavioral
health screenings in primary care settings and guidance on clinical
issues related to screening and referral. Implementation of five
commonly used screening tools - M-CHAT, PEDS, PHQ-9, PSC, and SDQ - is
highlighted.

CRAFFT Screening Tool
The
Massachusetts Department of Public Health, in conjunction with MBHP and
the Center for Adolescent Substance Abuse Research (CeASAR), has
published a tool kit for the CRAFFT (acronym for Car, Relax, Alone,
Forget, Friends, Trouble) screen. The CRAFFT screens for substance
abuse in adolescents.

CRAFFT screening tool forms are available in a wide variety of
languages including English, Chinese, Haitian Creole, French, Japanese,
