Integrated Care Management Program (ICMP)
The Integrated Care Management Program (ICMP) is an enhanced care
management program offered to Primary Care Clinician (PCC) Plan Members
with complex medical, mental health, and/or substance use disorders. Clinical staff provide
integrated medical and behavioral health care management which can
include direct, face-to-face care management visits with Members.

Who is Eligible for the ICMP?
A predictive modeling tool ana>lyzes behavioral health, medical, and
pharmacy claims data (historical and current) to identify Members with
the highest need for integrated physical and behavioral health services
and care coordination. The ICMP also accepts direct referrals from
providers and Members.
The ICMP staff will notify you if you are a PCC and have a Member(s)
in your panel identified for the ICMP and then again upon the Member's
enrollment into the program. For Members who may already be receiving
care management/coordination services in another program the ICMP staff
will work with you to ensure that there is coordination, not
duplication of services.

Certified Peer Specialists Can Help with Members' Care
A Certified Peer Specialist (CPS) has “lived experience.” They have had a mental health and/or substance use disorder or cared for a family member who has. Because of this,
the CPS can teach, offer support, and help Members and their family work with the healthcare system. Peers offer support without judgment. They provide hope that recovery is possible.
A CPS can help Members address issues such as:
- Developing a vision of wellness and recovery
- Developing a self-directed care plan
- Problem solving and goal setting
- Managing family relationships
- Finding community and social supports
- Locating non-traditional services
