Integrated Care Management Program (ICMP)

The Integrated Care Management Program (ICMP) is an enhanced care management program offered to Primary Care Clinician (PCC) Plan Members with complex medical, mental health, and/or substance use disorders. Clinical staff provide integrated medical and behavioral health care management which can include direct, face-to-face care management visits with Members. Who is Eligible for the ICMP?

A predictive modeling tool analyzes behavioral health, medical, and pharmacy claims data (historical and current) to identify Members with the highest need for integrated physical and behavioral health services and care coordination. The ICMP also accepts direct referrals from providers and Members.

How to Refer

Certified Peer Specialists Can Help with Members' Care

A Certified Peer Specialist (CPS) has lived experience. They have had a mental health and/or substance use disorder or cared for a family member who has. Because of this, the CPS can teach, offer support, and help Members and their family work with the healthcare system. Peers offer support without judgment. They provide hope that recovery is possible.
