Order Health Education Materials

The Massachusetts Behavioral Health Partnership (MBHP) offers free materials to help you learn about your physical and behavioral health. You can use the form below to print the materials.  Just click which language you want in the "Download as PDF" column.  Or, you can order the materials and we will send them to you.  Fill out your name and address below.  Then choose what materials you want us to mail to you by using the last column of the form.

You may also print this form and fax it with a cover page to: (855) 643-2340, attn: Ellen Youngclaus. If you have questions or need help, call us at 1-800-495-0086.

Please note, due to COVID-19 precautions, we are not in the office and are unable to fulfill materials order requests at this time. Electronic copies of MBHP materials are available for download below. For questions, contact MBHPInfo@carelon.com. We apologize for the inconvenience.
